Hospital-to-Home Program
Short-term, in-home support to ensure a safe transition from hospital or rehab to home, promoting recovery and reducing readmission risk. Caregivers provide compassionate assistance under RN guidance, helping clients regain independence safely.
Services Include:
RN-guided post-discharge assessment and personalized care plan
Daily support with activities of daily living (ADLs), including bathing, dressing, and mobility
Medication reminders and monitoring (non-administration)
Communication and coordination with physicians, therapists, and case managers
Education and guidance for family caregivers
Emotional support and companionship during recovery
RN / Licensed Professional Oversight:
RN visits (typically 1–2 per program) to review care plans and provide guidance to caregivers
Ensures clinical oversight while caregivers provide hands-on non-medical support
